OSHA requires you to report a recordable employee injury or illness within seven (7) calendar days of receiving information that a recordable injury or illness has occurred.

Here’s how you can record injuries and illnesses on OSHA forms on our recordkeeping website:

     The Data Entry Wizard will ask you each question you need to answer to complete each of your recordkeeping and reporting forms. As soon as you input your data and click "SAVE", the completed OSHA forms 301, 300, and 300A, and the Florida DWC-1, will be available for download.

     View Lost Workday Injury and Illness (LDWII) Rate, Total Recordable Case Rate (TRCR) and Days Away, Restricted or Transferred (DART) Rate for your incident data.
